To provide you with excellent customer service, we gather information from applications, or from other forms, through
telephone or in person, and/or internet search. We obtain name, address, telephone number, date of birth, gender, marital
status, occupation, prior insurance information, health history, payment preferences and other billing information.
From your transactions with CGF we also obtain payment history, rates, and information learned from researching claims
as approved on your behalf or on the behalf of your dependents and/or beneficiaries.

CGF does not sell, lease, or rent the personal information of our customers, potential customers, or former customers to
other companies. CGF does not provide personal information to persons or organizations for marketing purposes. CGF only
